- Are you the applicant, or a referring counselor?
- Is the applicant at least 18 years old?
- Can the applicant live independently with minimal assistance?
- Does the applicant currently have income or financial support?
- Is the applicant currently working with a case manager?
- What city or county does the applicant currently live in?
- Is the applicant a veteran?
- Is the applicant a senior citizen?
- Is the applicant currently experiencing homelessness or serious housing instability?
- Is there any additional housing or support information we should know?
