The Process
How admissions works
Regardless of how you enter the process — through a referral partner or by contacting us directly — everyone follows the same standardized path.
Step 1
Inquiry & Referral
Referred by a hospital, case manager, or community agency — or reach out directly. Every entry point leads to the same process.
Step 2
Pre-Screening
A short questionnaire to understand whether you appear to be a good fit — just a starting point, not a verdict.
Step 3
Eligibility Review & Intake
A deeper look at fit and readiness, followed by digital intake — information, documentation, and required acknowledgements.
Step 4
Staff Review & Interview
A standardized internal review. An interview may be scheduled if it would help clarify fit — it's a conversation, not an interrogation.
Step 5
Decision & Move-In Preparation
Approval, conditional acceptance, waitlist, or a respectful explanation if we're not the right fit right now — followed by move-in planning if approved.
Step 6
Orientation & Onboarding
Introductions, expectations, and ongoing check-ins as you settle in and begin working toward your goals.

What you'll need
Documentation requirements may vary and remain configurable as the program grows — your admissions contact will let you know exactly what applies to you.
- Government-issued photo identification
- Proof of income or financial support
- Benefit award letters (when applicable)
- Veteran documentation (when applicable)
- Referral and case management information
- Emergency contact information
Possible Outcomes
Every outcome is documented, fair, and respectful
Approved
You'll move into onboarding — agreement, remaining documents, and move-in planning.
Conditional Acceptance
Approved, with specific remaining requirements before move-in — you'll get a clear checklist and timeline.
Pending Additional Information
If something's missing, you'll know exactly what, how to submit it, and by when — this is not a denial.
Waitlist
Used only when timing — not eligibility — prevents immediate acceptance. You'll always know why, and what happens next.
